On February 8, 2021, the crew of an Embraer 500 Phenom 100 (registration 9H-FAM) experienced a loss of control of their aircraft shortly before attempting to land. This loss of control was due to the aircraft stalling as a result of ice contamination on its airframe. The impact upon landing led to the collapse of both the nose and main landing gear. The damage to the main landing gear resulted in a fuel leak, which subsequently ignited a fire as the aircraft skidded along the runway before veering off it. An investigation into the incident revealed that the flight had been conducted in conditions conducive to icing during the approach. However, the prescribed use of the airframe de-icing system had not been followed. Furthermore, it was noted that such non-compliance with de-icing procedures appeared to be a regular occurrence, with the associated risks not fully understood or appreciated.
